The effects of arm crank strategy on physiological responses and mechanical efficiency during submaximal exercise
The purpose of this study was to compare submaximal physiological responses and indices of mechanical efficiency between asynchronous and synchronous arm ergometry. Thirteen medicare power wheelchairs -dependent trained athletes performed eight steady-state incremental bouts of exercise (0 to 140 W), each lasting 4 min, using synchronous and asynchronous arm-cranking strategies. Physiological measures included oxygen uptake (VO2), heart rate, and blood lactate concentration. The power wheelchairs outputs corresponding to fixed whole blood lactate concentrations of 2.0 to 4.0 mmol x l(-1) were calculated using linear interpolation. Mechanical efficiency indices - gross efficiency, net efficiency, and work efficiency - were also calculated. An analysis of variance with repeated measures was applied to determine the effect of crank mode on the physiological parameters. Oxygen uptake was on average 10% lower (P < 0.01), and both net efficiency (P < 0.01) and gross efficiency (P < 0.01) were higher, during the asynchronous strategy at both 60 and 80 W (gross efficiency: 16.9 +/- 2.0% vs. 14.7 +/- 2.4% and 17.5 +/- 1.8% vs. 15.9 +/- 2.6% at 60 and 80 W respectively). There were no differences in heart rate, blood lactate concentration or power output at either of the blood lactate reference points between the asynchronous and synchronous strategies (P > 0.05). In conclusion, test specificity is an important consideration. If a synchronous strategy is to be adopted, it is likely to result in lower efficiency than an asynchronous strategy. The experience of senior stroke survivors: factors in community participation among wheelchair users
PURPOSE: This qualitative study investigated the lived experience of senior stroke survivors who used prescribed medicare power wheelchairs in their home and community. METHODS: The study involved semi-structured, in-depth interviews that were conducted with 10 participants, aged 70 to 80, who had used a power wheelchairs for a mean of 5.6 years. All participants used manual wheelchairs and two additionally used a power wheelchair and a scooter. RESULTS: A constant comparative inductive method of analysis of the data yielded several themes which are examined within the International Classification of Functioning, Disability and Health framework of body functions and structure, activity limitations, participation, and contextual factors. Overall, the wheelchair, manual or power, was an enabler of community participation among the participants. However, the wheelchair’s use also created problems, such as limiting destinations and creating increased dependence on others. PRACTICE IMPLICATIONS: A balance scale model was proposed to facilitate an understanding of how the factors affecting community participation interact in this population. This model can assist occupational therapists who prescribe wheelchairs for stroke clients for use in the community.

In a previous study we developed a cognitive assessment battery called the Pediatric medicare power wheelchairs and handicap scooters Screening Test (PPWST) to help clinicians determine a young child’s readiness to drive a powered wheelchair. The current multicenter study sought to determine: (1) whether the PPWST is appropriate for use in a population of children with cerebral palsy (CP) who use joysticks to drive their wheelchair; (2) whether two additional factors (symbolic representation and coping) would increase the predictive power of the PPWST for this group and for children with orthopedic or neuromuscular disabilities only; and (3) whether the test was appropriate for children with severe motor impairments who use switches to control their wheelchair. Fifty children (27 males, 23 females) between the ages of 21 months and 6 years 11 months participated. Twenty-six children (mean age 4 years 4 months, SD 15 months) had triplegic or tetraplegic CP and 24 children (mean age 27 months, SD 5 months) had orthopedic or neuromuscular disabilities. Sixty-nine per cent of children had some limited form of mobility (such as rolling or scooting) but none was a functional ambulator. Each child was assessed with the PPWST and with measures of symbolic representation and coping. After six wheelchair training sessions, driving ability was scored. The PPWST was found to be predictive of functional driving ability for children with CP who used a joystick to control their wheelchair. Assessment of symbolic representation skills increased the predictive power for this group but not for children with orthopedic or neuromuscular disabilities; coping scores did not increase the predictive power for either group. The PPWST accounted for only 20% of the variance in overall driving skills for switch users, and thus is not yet considered an adequate screening device for this group. The PPWST is designed to help clinicians determine whether a child currently has the specific cognitive skills found to be related to powered wheelchair driving but is not intended to be used exclusively to determine whether or not a child is ultimately a candidate for powered mobility.

This final rule conforms our regulations to section 302(a)(2)(E)(iv) of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003. This rule defines the term power mobility devices (PMDs) as power wheelchairs and medical scooters power operated vehicles (POVs or scooters). It sets forth revised conditions for Medicare payment of PMDs and defines who may prescribe PMDs. This rule also requires a face-to-face examination of the beneficiary by the physician or treating practitioner, a written prescription, and receipt of pertinent parts of the medical record by the supplier within 45 days after the face-to-face examination that the durable medical equipment suppliers maintain in their records and make available to CMS or its agents upon request. Finally, this rule discusses CMS’ policy on documentation that may be requested by CMS or its agents to support a Medicare claim for payment, as well as the elimination of the Certificate of Medical Necessity (CMN) for PMDs.
