In this study, we assessed the influence that pre-exercise glucose ingestion of two concentrations has on the physiological responses of paraplegic athletes. Eight men with paraplegia ingested a drink containing 4% (low) or 11% (high) carbohydrate in a randomized double-blind crossover design, 20 min before exercise. The participants performed medicare power wheelchairs exercise at 65% of peak oxygen uptake for 1 h followed by a 20 min performance test. During both trials, the physiological responses were similar and indicated steady-state exercise. At the onset of exercise, blood glucose concentrations in both trials increased after carbohydrate ingestion (P < 0.05) before returning to resting values after 20 min of exercise and there were no differences between trials. Free fatty acid concentrations increased from rest to 1 h of exercise in both trials, with a greater increase during the low carbohydrate trial that led to a difference in free fatty acids between trials at the end of the 1 h tests (P < 0.05). There was a tendency for the performance distances and power outputs achieved during the high carbohydrate trial to be greater than those achieved during the low carbohydrate trial (P= 0.08). In conclusion, when paraplegic athletes ingested low and high carbohydrate drinks before exercise, the decline in blood glucose concentrations was similar. The tendency for higher blood glucose concentrations, respiratory exchange ratios and medical scooters power outputs and lower free fatty acid concentrations (P < 0.05) during the high carbohydrate trial suggests that a higher concentration of carbohydrate in a sports drink might be a better choice for paraplegic athletes.

The range of a power wheelchairs and medicare power wheelchairs depends on many factors including: battery type, battery state, wheelchair/rider weight, terrain, the efficiency of the drive train, and driving behavior. The purpose of this study was to evaluate the feasibility of three methods of estimating power wheelchair range. Another significant purpose was to compare the current draw on pavement to current draw on an International Standards Organization (ISO) Double Drum tester at one m/sec. Tests were performed on seven different power wheelchairs unloaded, and loaded with an ISO 100 kg test dummy. Each chair was configured according to the manufacturer’s specifications, and tires were properly inflated. Experienced test technicians were used for the tennis court tests, and treadmill tests. An ISO 100 kg test dummy was used for the ISO Double Drum test. Energy consumption was measured over a distance of 1500 m for each of the three test conditions. The rolling surface was level in all cases. Repeated measure analysis of variance (ANOVA) revealed a significant difference (p = 0.0001) between the predicted range at maximum speed for the three tests. Post hoc analysis demonstrated a significant difference (p < 0.01) in estimated range at maximum speed between the Double Drum test and the treadmill test, as well as between the Double Drum test and the tennis court test. Our results indicate no significant difference (p > 0.05) between the predicted range at maximal speed between the treadmill and tennis court tests. A simple relationship does not exist between the results of range testing with the Double Drum tester and the tennis court. An alternative would be to permit the use of a treadmill for range testing as simple relationships between all pertinent treadmill and tennis court range data were found. For the Double Drum tester used, the current demand is higher than under normal usage. This presents a problem as current is related to load torque in a power wheelchair. Hence, the Double Drum tester friction must be reduced. The predicted range for the tennis court test at maximum speed ranges from a low of 23.6 km to a high of 57.7 km. The range of the power wheelchair can be improved by the use of wet lead acid batteries in place of gel lead acid batteries.

Acupuncture for chronic shoulder pain in persons with spinal cord injury: a small-scale clinical trial
OBJECTIVE: To determine the efficacy of acupuncture in the treatment of chronic musculoskeletal shoulder pain in subjects with spinal cord injury (SCI). DESIGN: Randomized, double blind (participants, evaluator), placebo (invasive sham) controlled trial. SETTING: Clinical research center. PARTICIPANTS: Seventeen manual wheelchair-using subjects with chronic SCI and chronic musculoskeletal shoulder pain. INTERVENTIONS: Participants were randomly assigned to receive 10 treatments of either acupuncture or handicap scooters, invasive sham acupuncture (light needling of nonacupuncture points). MAIN OUTCOME MEASURE: Changes in shoulder pain intensity were measured using the power wheelchairs User’s Shoulder Pain Index. RESULTS: Shoulder pain decreased significantly over time in both the acupuncture and the sham acupuncture groups (P=.005), with decreases of 66% and 43%, respectively. There was no significant difference between the 2 groups (P=.364). There was, however, a medium effect size associated with the acupuncture treatment. CONCLUSIONS: There appears to be an analgesic effect or a powerful placebo effect associated with both acupuncture and sham acupuncture. There was a medium treatment effect associated with the acupuncture, which suggests that it may be superior to sham acupuncture. This observation, along with the limited power, indicates that a larger, more definitive randomized controlled trial using a similar design is warranted

We are developing a Smart Wheelchair Component System (SWCS) that can be added to a variety of commercial power wheelchairs with minimal modification. This paper describes the design of a prototype of the SWCS, which has been evaluated on wheelchairs from four different manufacturers.

Loss of mobility due to lower limb paralysis is a common result of thoracic level spinal cord injury. Functional electrical stimulation (FES) can restore primitive gait in the vicinity of a wheelchair by using handicap scooters stimulation to generate muscle contractions. A new concept for FES-assisted gait is presented that combines electrical stimulation with an orthosis that contains a fluid power system to store and transfer energy during the gait cycle and medical scooters. The energy storage orthosis (ESO) can be driven through a complete gait cycle using only stimulation of the quadriceps muscles. The conceptual design of the ESO was completed and implemented in a dynamic simulation model and in a benchtop prototype for engineering measurements. No studies were conducted with human subjects. The results demonstrate the potential of the ESO concept for a feasible gait-assist system and the validity of the simulation model as a means for designing the system.

The group consensus was that durability and reliability are the most important criteria. At the same time, there is a desire to enhance and advance the features of input devices and control systems. Many would say these changes constitute designing “smarter” power wheelchairs, such as systems that can independently detect obstacles and can provide users with more feedback. This paper presents the rationale behind forming this focus group and details of the results of a brainstorming session where ideas were generated and prioritized. The five most important issues as determined by the group are discussed in depth.

Pressure, Are You Applying Enough to Attain Your Financial Freedom
One of the things that I have discovered in my journey of personal development is that I must
make the choice to step into success or I will just stand in failure. I find that it is
necessary to apply pressure to myself in order to move forward and take those steps into
success. Do you apply pressure to yourself? Let me ask you a question: do you wait until the
last minute to finish a task, project or chore? Are you one of those people who accomplish 70%
of the project in the last 10% of the time?’
If you said yes to either of those questions then you are applying pressure to yourself. Some
of us work better by doing little bits and pieces of the project, task or chore as we go.
Others work much better when they are under the gun. When they know they ‘gotta get it done’,
‘it’s due tomorrow’, ‘it’s due in two hours’.. Some people are much better when they are
applying that pressure to themselves.
We need to be able to recognize where we fall on that scale and realize how best we react
because if we don’t apply any pressure at all to ourselves then we are we are not going to
learn, grow and step up into the success that we want in our lives. We all want success in our
lives, whether it is emotionally, in relationships, with our family, in our

Many times, we human beings waste time feeling guilty. By doing this we are wasting our own
time and energy. When we get to the point of the deadline for that project, task or chore and
we’re feeling so guilty - ‘oh why didn’t I do something sooner? I should have spent more time!
Why did I spend my time on that thing and not get this project started?’ As human beings need
to learn that it is okay and to recognize that’s how we cope best, we work best under pressure.
So stop feeling guilty about it! Stop beating yourself up, because when we stop beating
ourselves up and wasting energy on the guilt, we have more time and more energy to invest in
the project, chore or task we want to complete.
The other thing that we do, in order to not apply the pressure, is we hide behind our
responsibilities. I used to do this so well - hide behind my responsibilities. Many times when
I have talked to people about a home based business opportunities in
Canada, they say ‘oh, well I don’t have the time to do something like that. I am so busy! I
have my family. I have my job. I have my charities. I have my hobbies.’ We all have those
different responsibilities in our life. But what we need to decide is ‘Is this
project/task/business something that I really want to do or rather is this
task/business/project going to create a result that I want. You have to begin by deciding what
result you want in your life and then arrange your time based on achieving those results. Ask
yourself : ‘Are there things I could rearrange or even drop all together to make time to do
this?’
Are you applying the pressure to your own life to do what it is you really want to do?
Sometimes those tasks or chores are not really things that we want to do, but we want that
specific end result so we do them. We want our relationships to work well. We want our
financial freedom. We want to be able to take our family to Disneyland. We want that specific
end result, the successful life. Today I just assisted my husband in setting up his financial
filing system, I sure did not want to spend time doing it, however I want the end result!! The
end result of the nice clean, tidy organized paperwork resulting in his taxes being completed
on time. I put the pressure on myself, assisted him and now I am very glad I did. Not only did
the paperwork issue get better but so did our relationship because my husband felt much
supported.
We need to focus on the results we want instead of hiding behind the excuses of ‘I don’t have
the time,I’ve got to go to work, I’ve got to take my daughter to soccer. I have got to take my
dog to the vet’. Yes, those are all things that need to be done. However when we decide that
achieving a certain task/chore/business is a priority that we could rearrange our time or give
up some unnecessary activities to free up time if we really wanted to. If we really want that
result, we could get it done, it just takes a decision. We will then stop investing in the
guilt. we will stop hiding behind the responsibilities and figure out a way to make it happen.
There is a universal law, like the law of gravity called the Law of Growth;. In order to exist,
we need to grow. How we grow is our choice. What seeds do we want to plant? Do we want to plant
the seeds of not getting something done? Or do we want the seeds of success habits, applying
the pressure and stepping into the successful life that we know we deserve. We all deserve a
successful life! Ask yourself “am I applying the correct pressure on myself to create my
successful life?” I hope you are!
